Hospice

Hospice and palliative care physicians must be able to facilitate transitions across the hospice and palliative medicine continuum of care, fulfill the role of the hospice medical director, and promote and teach about hospice. During the hospice rotation, fellows will learn about hospice in the home hospice setting, in a free-standing inpatient hospice facility, and in inpatient hospice settings within two tertiary care settings. During this time, fellows participate in interdisciplinary team meetings and begin to learn the skills required to be a hospice medical director.

This fellowship trains participants in the following Hospice and Palliative Medicine Core Competencies:

  • Recognizes the role of the interdisciplinary team in hospice
  • Describes the role of the palliative care physician in the interdisciplinary team
  • Demonstrates care that is cost-effective and represents best practices
  • Identifies similarities and differences between reimbursements for palliative medicine, hospice, hospital, home health, and long term care
  • Describes basic concepts and patterns of physician billing, coding and reimbursement across settings
  • Integrates knowledge of health care system in developing plan of care
  • Describes philosophy, admissions criteria, range of services, and structure of hospice care
  • Recognizes resources and barriers relevant to the care of specialized populations in hospice and palliative medicine, and has basic knowledge of how to mobilize appropriate support for these populations (e.g. pediatric patients, HIV patients, etc.)
  • Demonstrates knowledge of the various settings and related structures for organizing, regulating, and financing care for patients at the end of life
  • Describes differences in admission criteria for various settings such as hospitals, palliative care units, skilled-nursing and assisted-living facilities, acute/sub-acute rehab facilities, and long-term acute care settings as well as traditional home hospice
